By completing this Official Entry Form, paying the required entry fee, and signing this contract, I hereby enter the above Big Bass Splash® Tournament(s) - Little Anglers Division. In addition, I acknowledge that I have read, understand, and accept all the rules related to the tournament(s) and waive and release all other contestants, McDonald’s® Corporation, McDonald’s® of Jasper/Woodville, Texas; Leesville/DeRidder, Louisiana; Paris/Commerce/Sulphur Springs, Texas; Char, Inc; Guntersville/Scottsboro/Albertville/Boaz, Alabama; McWorth Management Company, Paris, TN; sponsors and their agents; Sealy Outdoors, Inc. and all tournament officials from any and all claims of personal injuries and accidents, as well as, any damaged, lost, or stolen property incurred in connection with the tournament(s). I understand that boat and liability insurance are my responsibility. I agree that I am not entitled to receive any royalties or other compensation in connection with such use.
